Our Health Survey is now live!
All eligible participants (see inclusion criteria below) can now complete the survey online.
A $40 Walmart gift card will be mailed to the address indicated on the consent form for fully completed surveys.
This survey is for people 16 and older who meet the following inclusion criteria:
- All registered and non-registered members of Nuu-chah-nulth Tribal Council member Nations residing anywhere in British Columbia, Canada.
- All individuals (Indigenous and non-Indigenous) living on-reserve in a Nuu-chah-nulth Tribal Council member Nation.
Please note: The survey takes between 30 and 60 minutes to complete online. If you cannot complete it in one sitting, you can save your progress and continue the survey at another time.
